Tell us about your experiences of mental health services
We are inviting you to share your experiences of the mental health services provided by Camden and Islington NHS Foundation Trust.
This anonymous survey, which takes about 15 minutes to complete, has been requested by Councillors in the Health Scrutiny Committee at Camden Council.
Your views are really valuable and will help to develop local services. Your responses will be completely anonymous and all information will be kept confidential and not shared with anyone outside of Public Health.
After completing the survey, you have the option to provide your name and contact details to be entered into a prize draw for one of three £20 gift vouchers or for participation in a follow-up group discussion. However, the findings which will be shared afterwards will NOT include your name or any contact details.
